Transconjunctival Approach With Pedicled Orbital Septal Fat Flap Repositioning for Correction of Tear Trough Deformity.

Lower eyelid plasty is important for facial rejuvenation. Tear trough deformities and eyebags are among the most common and urgent problems encountered in lower eyelid aesthetic surgery. Compared with fillers such as hyaluronic acid and fat particles, a pedicled orbital septal fat flap for the correction of tear trough deformities has several advantages, including a higher survival rate, biocompatibility, and long-term results. To simultaneously solve the problem of eyebags combined with tear trough deformities, we performed eyebag surgery while releasing the orbital septal fat, trimming it to form 2-3 clusters of pedicled orbital septal fat flaps, and separating the fat flaps vertically at the orbital rim periosteum to reset the cavity and correct the tear trough deformities. This technique meets the needs of today's youthful aesthetics, effectively avoids complications such as lid ectropion caused by the external incision approach, and has reliable safety and efficacy with satisfactory cosmetic results.
